SoCal Home Health

For Area Agencies on Aging and ADRCs

Comparison charts and eligibility sheets your staff can hand a client during the call.

Your team already knows this material. What is usually missing is something to put in the client's hand at the end of the conversation, in the language they actually read.

What you can use

  • Side-by-side comparison of Medicare, Medi-Cal, IHSS and VA benefits
  • Eligibility checklists a client can work through at home
  • Program finder and cost tools that work on a phone with no account
  • Guides on HCBS waivers, PACE and Medi-Cal for long-term care

Printable handouts

Print these, hand them out, put them in a newsletter, or leave them on a table. Every one is available in all eight languages we publish.

Available in English, Spanish, Armenian, Persian, Filipino, Korean, Vietnamese and Chinese.

Common questions

Can we use these alongside our own materials?

Yes — they are meant to supplement, not replace. Print them, adapt them into your own newsletter, or link to them.

Do you cover programs outside home health?

Yes. IHSS, CAPI, PACE, HCBS waivers, Paid Family Leave, CalFresh, property tax postponement and utility help are all covered, because families rarely need only one thing.

Who funds this?

A Medicare-certified home health provider serving Southern California. That is stated on the funding disclosure page. The educational content is not conditioned on anyone using them.
