For physician practices and referral coordinators
You identify the patient. We handle the insurance, the authorization and the scheduling, and the notes come back to your chart.
Most of the delay between "this patient needs home health" and a nurse actually being in the house is paperwork your front desk should not have to do. That part is ours.

Common questions
What do you need to start a referral?
Patient name, date of birth, a working phone number, insurance, the ordering physician, and a short reason or a recent visit note. A face sheet covers most of it if that is easier.
Which insurance do you accept?
Primarily Medicare and veterans coverage (VA and TRICARE West/TriWest). Ask about anything else and we will tell you honestly before the patient is contacted.
How fast is the patient contacted?
Within 24 hours of receiving the referral, and we confirm receipt with your office.
What comes back to my office?
Clinical notes for the patient's record. You keep visibility without chasing anyone for it.
